Job Summary:

Care for individuals with intellectual and developmental disabilities in a day program setting. Implement and monitor program plans to promote independence, individualization, and productivity.

Responsibilities:

  • Monitor activities and ensure consistency with valued outcomes
  • Provide direction to habilitation assistants
  • Maintain daily documentation and use software for data collection
  • Assist with feeding, personal hygiene, and lifting/transferring as needed
  • Ensure health, safety, and well-being of program participants
  • Maintain equipment and ensure a clean and safe environment
  • Notify management and/or nurse of medical concerns and accompany participants to medical appointments as needed

Requirements:

  • Associates degree or 60 credits in a healthcare or educational field
  • 1 year experience working with individuals with intellectual and/or developmental disabilities
  • Excellent communication and interpersonal skills
  • Ability to work outdoors for community-based activities
  • Proficiency in data entry and MS Word
  • Criminal background check and valid NYS driver's license preferred
  • Successful completion of a medication administration program
