Habilitation Specialist

At Constructive Partnerships Unlimited, we are seeking a skilled Habilitation Specialist to join our team of professionals at our Queens Long Island City Day Program.

Job Summary:

The Habilitation Specialist will be responsible for implementing and monitoring program plans for individuals with intellectual and developmental disabilities. This includes ensuring the activities and training provided are consistent with valued outcomes of independence, individualization, and productivity.

Key Responsibilities:

• Implement community-based training opportunities, including use of public transportation, travel training, and safety skills.
• Provide direction to Habilitation Assistants and ensure daily documentation is maintained.
• Work directly with individuals attending the program, assisting with feeding, personal hygiene, and lifting/transferring as needed.
• Ensure the health, safety, and well-being of all individuals attending the program.
• Notify management and/or nurse of any medical concerns and accompany individuals to the hospital and/or healthcare provider as needed.

Requirements:

• Associates Degree/60 credits in a Health Care or Educational field.
• 1 year experience working with individuals with intellectual and/or developmental disabilities.
• Excellent communication and interpersonal skills.
• Ability to work outdoors for community-based activities as needed.
• Ability to perform data entry on a personal computer (PC).
• Valid NYS Driver's License preferred.
