Habilitation Specialist

At Constructive Partnerships Unlimited, we are seeking a dedicated Habilitation Specialist to join our team of professionals at our Queens Long Island City Day Program.
The Habilitation Specialist will be responsible for monitoring the activities of individuals attending the day program and ensuring that they are consistent with valued outcomes of independence, individualization, and productivity.
The ideal candidate will have an Associate's Degree in a Health Care or Educational field and at least one year of experience working with individuals with intellectual and/or developmental disabilities.
Key responsibilities will include:
- Implementing and monitoring program plans for individuals with intellectual and developmental disabilities
- Providing direction to Habilitation Assistants
- Maintaining daily documentation and using various software for data collection
- Ensuring the health, safety, and well-being of all individuals attending the program
- Notifying management and/or nurse of any medical concerns and accompanying individuals to the hospital and/or healthcare provider as needed
