Job Summary:

  • Estimated Salary: $65,000 - $85,000 per year, depending on experience
  • This is a full-time position, working Monday through Friday, 8:00am to 4:30pm

Job Description:

We are seeking a detail-oriented and organized Medical Care Coordinator to join our team. The ideal candidate will have excellent communication skills and be able to work effectively with patients, providers, and other healthcare professionals.

Responsibilities:

  • Coordinate patient care activities, including preparing patient records and ensuring that necessary information is communicated to providers and staff
  • Assist with triage, responding to patient inquiries, and coordinating referrals to specialists or other services as needed
  • Maintain accurate and up-to-date patient records, including demographic information, medical history, and treatment plans
  • Provide education and support to patients and families regarding health conditions, treatments, and self-care practices
  • Collaborate with multidisciplinary teams to ensure seamless transitions of care and optimal patient outcomes

Requirements:

  • Bachelor's degree in nursing or related field
  • Current licensure as a Registered Nurse (RN) or Licensed Practical Nurse (LPN), issued by the PA State Board of Nursing
  • AHA CPR/BLS certification required within 30 days of hire
  • 1-2 years of experience in a primary or specialty care physician's office, preferably with experience in triage, emergency room, cardiac care, intensive care, or other related specialties

Preferred Qualifications:

  • Prior experience with electronic health records and practice management systems
  • Excellent communication and interpersonal skills, with ability to work effectively with diverse populations
