About the Job

We are seeking a skilled Clinical Medication Specialist - Patient Care Coordinator to join our team at Lancaster General Health. This is an exciting opportunity for a dedicated professional to make a meaningful impact in patient care.

Job Summary

This role involves collecting and entering patient medication histories into our hospital's information system, ensuring accurate and timely data entry for healthcare professionals. As a Clinical Medication Specialist, you will effectively communicate with patients and other healthcare staff to maintain smooth patient flow and address individual needs.

Main Responsibilities
  • Maintain Accurate Patient Records: Collect patient medication lists from various sources and accurately enter them into our computer system.
  • Effective Communication: Collaborate with healthcare professionals to identify patient-specific needs and resolve issues promptly.
  • Competency in Medication Safety: Demonstrate expertise in using medication safety technology, including pyxis and barcode scanning.
  • Screening Assessments: Conduct assessments for drug and alcohol, suicide, Hep C, pneumococcal, and influenza, and place orders as needed.
Requirements
  • Licensure and Education: High school diploma or GED equivalent, LPN or RN licensure, or Pharmacist with vaccine licensure.
  • General Computer Skills: Proficiency in general computer skills and software applications.
  • Knowledge of Medications: In-depth knowledge of medication names, uses, doses, and routes of administration.
  • Excellent Communication Skills: Strong written and verbal communication skills for effective patient interaction.
