Medical Case Manager

Key Responsibilities:- Utilize advanced clinical judgment and critical thinking skills to facilitate appropriate member physical health and behavioral healthcare through assessment and care planning, direct provider coordination/collaboration, and coordination of psychosocial wraparound services.
- Telephonic clinical case management with Medicare population.
- Complete a Comprehensive Assessment and Plan of care.
- Document in clinical systems to support legacy Aetna and Coventry membership.
- A Valid unrestricted independent professional behavioral health clinical license to practice per state regulations in the state you reside in/one or more or equivalent (ex: LPC, LMFT, LCSW, LMHC, LCMHC, LICSW, LISW, Clinical Psychologist).
- Managed care/utilization review experience preferred.
- Case management and discharge planning experience preferred.
- Masters Degree in Social Work or Counseling required.
