Job Summary

We are seeking a skilled Mental Health Outreach Counselor to join our team at ServiceNet. As a key member of our multi-disciplinary team, you will provide support and guidance to individuals in their recovery journey, promoting resilience, hope, and recovery.

Responsibilities
  • Assist participants in achieving their individualized recovery goals and objectives.
  • Collaborate with the team to develop treatment plans, supporting participants' involvement and reviewing identified goals and tasks on a weekly basis.
  • Use evidence-based strategies and interventions to restore and teach daily living skills, promoting safe housing, self-care, work, education, social relations, and financial stability.
  • Provide monitoring and support to maintain a safe living environment for assigned individuals.
  • Transport participants as assigned by program supervisors.
  • Complete all required documentation.
  • Attend weekly team meetings and other scheduled meetings and trainings.
  • Assist in MHRS residential programs as needed.
Qualifications
  • Bachelor's degree preferred.
  • High School Diploma / GED.
  • Experience working with individuals in mental health recovery.
  • Valid driver's license and motor vehicle record.
