Job Summary

We are seeking a highly skilled Nurse Practitioner or Physician Assistant to join our team at Landmark Health, a part of Optum. As a member of our interdisciplinary team, you will provide high-quality, patient-centered care to our most medically vulnerable patients in the comfort of their own homes.

Key Responsibilities
  • Provide Comprehensive Care: Conduct initial comprehensive assessments, develop patient management care plans, and provide ongoing care to patients with complex medical needs.
  • Collaborate with Interdisciplinary Team: Work closely with our team of healthcare professionals, including physicians, nurses, and social workers, to ensure seamless care coordination and communication.
  • Prescribe Appropriate Diagnostics and Interventions: Use your clinical expertise to prescribe diagnostics and interventions that promote patient health and prevent unnecessary hospitalizations.
  • Stay Up-to-Date with Clinical Standards: Stay current on relevant medical and nursing research, technology, and related issues by attending continuing education courses, professional meetings, and journal reviews.
Requirements
  • Completion of Nurse Practitioner or Physician Assistant Program: Must have completed an accredited program in nurse practitioner or physician assistant studies.
  • Current State RN License: Must have a current state RN license.
  • Current NP/PA License: Must have a current NP/PA license.
  • Access to Reliable Transportation: Must have access to reliable transportation and comply with Optum's Motor Vehicle Safety policy.
