We are seeking a skilled Medical Provider to join our team at Optum. As a Nurse Practitioner, you will play a vital role in delivering high-quality home-based medical care to our patients.
As a member of our interdisciplinary team, you will work closely with patients, caregivers, and other healthcare professionals to develop and implement personalized care plans. Your expertise will be essential in providing comprehensive assessments, diagnosing conditions, and prescribing treatments.
Key Responsibilities:
- Conduct initial comprehensive assessments and ongoing care for patients
- Develop and update patient management care plans as needed
- Collaborate with patients, caregivers, and healthcare professionals to ensure seamless care delivery
- Prescribe diagnostics and interventions to prevent unnecessary hospitalizations
- Perform basic procedures, including IV placement, blood draws, and wound care
Requirements:
- Completion of Nurse Practitioner or Physician Assistant accredited program
- Current state RN license and NP/PA license
- Access to reliable transportation
