We are seeking a highly skilled Director of MDS to join our team at North Shore Manor. As a key member of our staff, you will be responsible for conducting and coordinating the development, completion, and transmission of the resident assessment in accordance with state requirements and facility policies.

About the Role:

  • Salary: $65,000 - $85,000 per year, depending on experience
  • Location: [Assumed Location]

Key Responsibilities:

  • Conduct preliminary and comprehensive resident assessments (MDS) in accordance with current rules, regulations, and guidelines
  • Ensure care plans include measurable objectives and timetables to meet residents' needs
  • Coordinate interviewing of each resident for the assessment
  • Develop and update comprehensive resident assessments and care plans
  • Evaluate residents' conditions and pertinent medical data

Requirements:

  • 2-4 years' experience providing direct care to multiple patients/residents; previous experience in Skilled Nursing environment desirable
  • Nursing degree from accredited college or graduation from approved LPN or RN program
  • Current, unencumbered license to practice as an LPN in Colorado; RN license preferred
