Hospice Nurse Case Manager

We are seeking a compassionate and skilled Registered Nurse Case Manager to join our team at Empyrean Hospice. As a Christian provider, we offer a unique opportunity to make a meaningful difference in the lives of our patients and their families.
Responsibilities- Represent Empyrean Hospice with professionalism and compassion, providing exceptional care to patients and their families.
- Conduct comprehensive assessments of patients and their families to determine their needs and develop individualized care plans.
- Coordinate patient care, ensuring seamless transitions and continuity of care.
- Counsel patients and their families on meeting their nursing and related needs, providing health care instructions as appropriate.
- Assist patients with activities of daily living and facilitate their efforts toward self-sufficiency and comfort care.
- Document patient care accurately and thoroughly, maintaining comprehensive clinical notes.
- Communicate effectively with physicians and the hospice care team, reporting changes in patient condition and obtaining/receiving physician orders as required.
- Provide a safe environment for patients, arranging equipment and services as needed.
- Supervise ancillary personnel and delegate responsibilities when required.
- Professionalism, compassion, empathy, and a desire to help others.
- Self-directing with the ability to work with little direct supervision, yet also as a member of a complete interdisciplinary care team.
- Understanding of the philosophy of hospice care.
- Current licensure in State and CPR certification.
- Graduate of an accredited school of nursing with 1-2 years of recent acute care experience in an institutional setting, and 1-2 years of recent experience in home care.
- Bachelor's degree, with one year of hospice care experience preferred.
- Management experience not required, but will be responsible for supervising hospice aides.
- Excellent observation, verbal and written communication skills, problem-solving skills, basic math skills; nursing skills per competency checklist.
- Able to lift, position, and/or transfer patients, and able to lift supplies and equipment.
- Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order.
- Must possess a willingness to maintain comprehensive working knowledge regarding information systems and applicable software programs.
