Empyrean Hospice Registered Nurse Case Manager Job Description

We are seeking a compassionate and dedicated Registered Nurse Case Manager to join our team at Empyrean Hospice. As a Christian hospice provider, we offer the highest quality clinical care and the love and salvation of Jesus Christ.

Job Summary

The Registered Nurse Case Manager will be responsible for planning, organizing, and directing hospice care with an emphasis on community health education and experience. This role requires a strong understanding of the philosophy of hospice care and the ability to work with patients and families to determine their needs and develop individualized care plans.

Key Responsibilities
  • Represent Empyrean Hospice with professionalism and compassion
  • Conduct comprehensive assessments of patients and families to determine their needs and develop individualized care plans
  • Coordinate patient care for assigned caseloads
  • Counsel patients and families on meeting nursing and related needs
  • Provide health care instructions to patients as appropriate
  • Assist patients with activities of daily living and facilitate their efforts toward self-sufficiency and optional comfort care
  • Complete and maintain accurate and relevant clinical notes regarding patient condition and care given
  • Communicate with physicians and hospice care teams regarding patient needs and report changes in patient condition
  • Provide and maintain a safe environment for patients and arrange equipment and other necessary items and services as needed
  • Supervise ancillary personnel and delegate responsibilities when required
Requirements
  • Professionalism, compassion, empathy, and a desire to help others
  • Self-directing with the ability to work with little direct supervision
  • Current licensure in State and CPR certification
  • Graduate of an accredited school of nursing with 1-2 years of recent acute care experience in an institutional setting and 1-2 years of recent experience in home care
  • Bachelor's degree with one year of hospice care experience preferred
  • Management experience not required, but will be responsible for supervising hospice aides
  • Excellent observation, verbal and written communication skills, problem-solving skills, basic math skills, and nursing skills per competency checklist
  • Able to lift, position, and/or transfer patients and able to lift supplies and equipment
  • Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order
  • Must possess a willingness to maintain comprehensive working knowledge regarding information systems and applicable software programs
