Key Responsibilities:
  • Assess patients' health needs and develop personalized care plans to improve health outcomes.
  • Provide education and coaching to patients and their families on healthy behaviors, disease management, and self-care.
  • Collaborate with healthcare teams to ensure seamless transitions of care and coordinate services to meet patients' needs.
  • Track patient outcomes and identify areas for improvement to inform care planning and population health initiatives.
Requirements:
  • NYS Registered Nurse (RN) license.
  • Bachelor's degree in nursing or related field.
  • Minimum of two years of experience in primary care, patient coaching, or outpatient case management.
  • Excellent communication and interpersonal skills to work effectively with patients, families, and healthcare teams.
Working Conditions:

This role will be based in an office/clinical setting with occasional travel to patient homes or community locations.
