Job Summary:

As a Care Coordinator Residential Specialist, you will be responsible for providing direct care services to children residing in a residential group home. Your primary focus will be on assisting consumers with their daily needs, including basic necessities, meal planning, activities, and housekeeping. Additionally, you will work closely with guardians and consumers to develop person-centered plans and monitor progress towards goals.

In this role, you will also be responsible for maintaining accurate and timely documentation of consumer needs and services provided. Responsibilities:
  • Provide direct care services to children in a residential group home setting
  • Assist consumers with daily needs, including basic necessities, meal planning, activities, and housekeeping
  • Develop and implement person-centered plans in collaboration with guardians and consumers
  • Maintain accurate and timely documentation of consumer needs and services provided
  • Ensure services align with North Carolina DD/MH/SAS standards
  • Participate in clinical training sessions and administrative staff meetings
  • Engage in quality improvement activities to enhance clinical skills
Requirements:

To be successful in this role, you will need a high school diploma or equivalent and relevant experience working with vulnerable populations. You must possess excellent communication and interpersonal skills, as well as the ability to remain calm and composed in challenging situations. A strong commitment to delivering exceptional care and support is essential.
