About the RoleAs a HouseCalls Clinician, you will play a vital role in delivering comprehensive, patient-centered care to individuals across Arizona. Your expertise will be invaluable in identifying and addressing health risks, coordinating follow-up care, and improving overall well-being.
Key Responsibilities- Conduct in-home assessments, including medical history reviews, medication reconciliation, vital signs, and comprehensive physical exams
- Perform evidence-based practice screenings, including point-of-care testing as needed
- Formulate diagnoses and develop care management plans to inform members' primary care physicians of potential gaps in care
- Communicate findings and educate patients on disease processes, medication, and compliance
- Travel up to 85% of the time, with all travel expenses covered
- Active NP or PA license or ability to obtain by start date
- Active national NP or PA certification or ability to obtain national certification in the state of assignment
- Ability to complete physical requirements of the job, including lifting a 30-pound bag and navigating various dwelling conditions
- Reliable transportation for home visit assessments
- 1+ years of clinical experience in a related field
- Experience in Gerontology, Cardiology, Internal Medicine, or Endocrinology
- Home health care or home visit experience
