Job DescriptionWe are seeking a skilled Licensed Practical Nurse to join our team at the Postgraduate Center for Mental Health. As a Licensed Practical Nurse - Mental Health Specialist, you will play a critical role in providing high-quality care to individuals with serious mental illness in the Bronx.
**Job Scope:**
This is a direct service position requiring specific knowledge of mental health, and skills in crisis intervention and stabilization. As part of our team, you must be willing to go out into the community to provide services. This position requires clinical experience, and the desire to work as part of a team.
**Responsibilities:**
- Participate in daily morning meetings to discuss patient care and treatment plans.
- Assist the RN with administering IM's and preparing oral medications for weekly delivery by the clinical team.
- Assist the RN with maintaining monthly IM calendars.
- Coordinate with team prescribers and pharmacy to ensure consumers receive medications and refills.
- Transcribe orders and update Medication Administration Records.
- Assist with logging in medication deliveries and completing toxicology request forms.
- Assist with completion of annual nursing assessments and daily progress notes.
- Conduct weekly Healthy Living groups with consumers.
- Perform other duties consistent with the goals and objectives of the program, as directed by the Team Leader.
**Requirements:**
- Current New York State LPN License.
- Current CPR & First Aid Certificate.
- Experience serving the mentally ill population.
**Salary:** $58,000-$60,000 annually
**Scheduled Shift:** Monday-Friday 9AM to 5PM
