Job Summary

We are seeking a dedicated Registered Nurse to join our team as a Nurse Care Coordinator for the Intermediate Care Unit. In this role, you will provide high-quality patient care while collaborating with a multidisciplinary team to achieve optimal patient outcomes.

About the Role

This position requires a strong foundation in nursing principles and practices, with a focus on critical care and intermediate care. You will assess, diagnose, and develop individualized plans of care for patients with complex medical conditions, including those who have suffered strokes. Your expertise will be crucial in coordinating care with other healthcare professionals to ensure seamless transitions and optimal patient experiences.

In addition to providing exceptional patient care, you will also play a key role in educating patients and their families about their conditions, treatment options, and self-care strategies. Your excellent communication and interpersonal skills will facilitate collaboration with the care team, ensuring that all stakeholders are informed and engaged throughout the care process.

Your professional judgment, critical thinking skills, and commitment to evidence-based practice will guide your decision-making, enabling you to provide compassionate, patient-centered care that prioritizes each individual's unique needs and goals.

Responsibilities
  • Assess patients' health care needs and develop individualized plans of care in collaboration with the care team
  • Implement evidence-based interventions to achieve desired patient outcomes
  • Carefully coordinate patient transfers between units and departments to ensure smooth transitions
  • Provide education and support to patients and their families to promote healthy behaviors and self-management
  • Document patient information accurately and thoroughly in electronic medical records
  • Collaborate with other healthcare professionals to optimize patient care and achieve shared goals
Requirements

To be successful in this role, you will need:

  • Diploma or Associate's degree in Nursing (ADN) required; Bachelor of Science in Nursing (BSN) or Master's of Science in Nursing (MSN) preferred
  • Current Oklahoma Registered Nurse license; Basic Life Support (BLS) training or retraining is required during orientation prior to providing patient care
  • Prior experience in healthcare field preferred; new graduates will be required to complete Norman Regional's Nurse Residency Program

  • Shift differentials available: $2.50/hour for evenings, $3.00/hour for overnights, and $3.00/hour for weekends
