Job Summary

Carilion Clinic is seeking a skilled RN Case Manager to join our Home Health team. As a RN Case Manager, you will be responsible for managing nursing care and providing individualized care for patients of all ages. You will utilize delegation and critical thinking skills to provide quality service through a team-oriented, collaborative approach to patient care.

Key Responsibilities
  • Interact with patients, families, visitors, and all members of the healthcare team to develop, coordinate, and manage the plan of care.
  • Manage and oversee delivery of patient care, including maintaining patient, staff, and customer confidentiality.
  • Demonstrate knowledge of the principles of life span growth and development and assess data reflective of the patient's status.
  • Interpret the appropriate information needed to identify each patient's requirements relative to their age-specific needs and provide care according to departmental policies and procedures.
  • Work involves application of advanced knowledge and skills of patient care, with duties that are varied and complex.
  • Accountable to resolve issues as determined by license, handling routine issues and following up to conclusion on matters, while referring only the most complex or higher-level issues and problems to the Manager, Team Lead, or Supervisor.
  • Must be able to work in a fast-paced environment with the ability to effectively prioritize and demonstrate multi-tasking skills.
  • Impact of work affects CHCS and Carilion Clinic scorecard initiatives, requiring strong critical thinking skills to interpret laws and regulations related to CHCS and Carilion Clinic.
  • May be exposed to infectious diseases, bio-hazardous, and hazardous materials, including regulated medical wastes, and combative patients.
  • Work is performed in a patient care environment and is often fast-paced, requiring use of prioritization skills.
  • Physical requirements include lifting, prolonged periods of standing, bending, pushing, pulling, reaching, walking, etc.
  • May be asked to provide care at other Carilion facilities to meet patient needs, with some positions requiring call and travel in adverse weather.
Requirements
  • Education: Graduate of a school of professional nursing.
  • Experience: One year of experience preferred.
  • Licensure, certification, and/or registration: Currently licensed as a Registered Nurse in the Commonwealth of Virginia or new graduate of an approved program eligible to sit for the Virginia licensure exam.
  • For Home Health, must have a valid Virginia Driver's License and pass a Criminal Record Check as per Virginia state law.
  • Must obtain/maintain AHA BLS-HCP.
  • Other Minimum Qualifications: Must successfully complete orientation and competency requirements, with keyboarding skills required for automated computer systems, including electronic medical records.
