Job Title: RN Case Manager - Home Health

Join Carilion Clinic, a regional leader in healthcare, as a RN Case Manager - Home Health. As a key member of our team, you will be responsible for managing nursing care and providing individualized care for patients of all ages.

Key Responsibilities:
  • Interact with patients, families, visitors, and healthcare team members to develop, coordinate, and manage patient care plans.
  • Maintain patient, staff, and customer confidentiality, while demonstrating knowledge of life span growth and development.
  • Assess patient data to identify requirements relative to age-specific needs and provide care according to departmental policies and procedures.
  • Work in a fast-paced environment, prioritizing tasks and demonstrating multi-tasking skills to impact CHCS and Carilion Clinic scorecard initiatives.
  • May be exposed to infectious diseases, bio-hazardous materials, and combative patients, requiring the ability to handle complex issues and refer to Manager, Team Lead, or Supervisor as needed.
Requirements:
  • Graduate of a school of professional nursing.
  • One year of experience preferred.
  • Currently licensed as a Registered Nurse in the Commonwealth of Virginia or new graduate eligible to sit for the Virginia licensure exam.
  • Valid Virginia Driver's License and pass a Criminal Record Check as per Virginia state law.
  • AHA BLS-HCP certification.
