Job Summary

We are seeking a skilled RN Case Manager to join our Home Health team in Radford, VA. As a key member of our healthcare team, you will be responsible for managing nursing care and providing individualized care for patients of all ages.

Key Responsibilities
  • Develop, coordinate, and manage patient care plans in collaboration with the healthcare team.
  • Provide high-quality patient care, including assessment, planning, implementation, and evaluation of patient outcomes.
  • Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination.
  • Maintain accurate and up-to-date patient records, including medical histories, treatment plans, and progress notes.
  • Participate in quality improvement initiatives to enhance patient care and outcomes.
Requirements
  • Graduate of an accredited school of professional nursing.
  • One year of experience in home health or a related field preferred.
  • Current licensure as a Registered Nurse in the Commonwealth of Virginia or eligibility to sit for the Virginia licensure exam.
  • Valid Virginia driver's license and ability to pass a criminal record check.
  • AHA BLS-HCP certification.
