About the Position

We're seeking a skilled Nurse Practitioner to join our team, providing primary, urgent, and acute medical care to patients in their homes. As a member of our care team, you'll work closely with our physicians, social workers, and other clinicians to deliver high-quality care and support to our patients and their families.

Responsibilities
  • Provide primary care to patients in their homes, including initial screenings, physical examinations, and ongoing care
  • Collaborate with our care team to develop and implement patient care plans
  • Order and interpret diagnostic tests, and determine the need for follow-up appointments and further treatment options
  • Prescribe and manage medications and treatments as appropriate
  • Update patient records and ensure accurate documentation
  • Deliver palliative care and end-of-life care to patients and their families, as needed
