We're currently hiring for FT Day/Eve - 12hr shift positions.
Job Responsibilities- Provides basic nursing care by assisting in the assessment of the resident on admission and on an ongoing basis.
- Remains knowledgeable about and communicates to the appropriate interdisciplinary team member any significant change in condition.
- Accurately documents all assessments.
- Participates in the care planning process for the resident.
- Updates the care plan when specific interventions indicate the need for revision.
- Communicates the plan to the appropriate IDT members along with the residents and their families.
- Education from an accredited school of nursing that results in becoming a Licensed Practical Nurse.
- Current LPN license in the state you will be working.
- CPR and AED certification required, or willing to obtain within six months of employment.
- IV certification required, or willing to obtain within six months of employment.
- Long-term care experience preferred.
