Job Summary

WellBe Senior Medical is seeking a highly skilled Patient Care Coordinator to join our interdisciplinary team. As a Patient Care Coordinator, you will play a vital role in providing compassionate care to our frail and elderly patients who suffer from chronic and acute illnesses or injuries.

Key Responsibilities
  • Coordinate care with the medical team, scheduling patient appointments, specialist appointments, and diagnostic tests.
  • Act as a community resource for patients, providing support and guidance to ensure they receive the best possible care.
  • Update and maintain digital client records, ensuring accurate and complete information is available to the care team.
  • Manage patient medical records, prioritizing urgent needs and scheduling needs for Complete Care Assessment.
  • Provide positive customer service, treating patients and staff with respect and dignity.
  • Collaborate with the interdisciplinary team, participating in regularly scheduled team meetings and contributing to quality of care efforts.
  • Support Advanced Practice Clinicians (APCs) in ongoing panel management and quality of care efforts.
Requirements
  • High school graduate or GED
  • 2+ years of experience working in a healthcare practice or hospital
  • EMR documentation experience preferred
  • Strong critical thinking skills, customer centric focus, and strong computer skills
  • Ability to lift up to 20lbs, stand for extended periods, and drive to patient locations
