We are seeking a skilled Pharmacist to join our team in a long-term care setting. The ideal candidate will have a strong background in medication management and a passion for delivering high-quality patient care.

Key Responsibilities
  • Evaluate physician medication orders for appropriateness and make recommendations for therapy changes as needed.
  • Maintain accurate and up-to-date patient records, including current and past medical history, clinical and laboratory data, and concurrent medications.
  • Collaborate with the healthcare team to provide clinical and technical support, ensuring the safe and effective use of medications.
  • Implement policies and procedures for the procurement, storage, distribution, and use of medications, as well as disposal of expired or unused medications.
  • Communicate effectively with physicians, nurses, and other healthcare team members to ensure seamless care delivery.
Requirements
  • Bachelor's Degree in Pharmacy or PharmD
  • 1+ year experience in long-term care or a related field
  • Valid/Active Pharmacist license in Georgia
  • Knowledge of pharmacy laws and regulations, including federal and state guidelines
