Staff Nurse-PAC Part-Time

NYC Health + Hospitals/Carter is a state-of-the-art facility with a 5-Star CMS Quality Rating, equipped with 164-bed Skilled Nursing Facility and 201-bed Long-Term Acute Care Hospital (LTACH) based in Harlem. Our team excels at providing quality medical, sub-acute, rehabilitative and long-term specialty services for individuals in need of medically-complex care like ventilator weaning.
Key Responsibilities:- Provide culturally competent, patient-centered, professional nursing care with dignity, compassion, and respect to the diverse resident population.
- Develop, implement and continuously evaluate the nursing care plan for assigned residents.
- Adhere to the nursing process when providing care/services to residents (assessment, nursing diagnosis, planning, implementation, and evaluation).
- Formulate and implement comprehensive, person-centered nursing care plan. Set priorities, deliver and direct care to meet resident needs.
- Participate in quality assurance performance improvement activities, Shared Governance, Clinical Ladder Program, Safe Patient Handling, Pathways to Excellence, and other committees as assigned.
- A valid New York State license and current registration to practice as a Registered Professional Nurse in New York State.
- Holds, or obtains through facility orientation, a valid and current certification in Basic Life Support (BLS) through the American Heart Association (AHA).
