NYC Health + Hospitals/Gouverneur is a 295-bed Skilled Nursing Facility (SNF) with a 5-Star CMS Quality Rating located in Manhattan's Lower East Side neighborhood. Our facility offers enhanced, cutting-edge healthcare services in a state-of-the-art space. We provide 24-hour care for individuals in need of sub-acute care, long-term care, or hospice care, including services like physical therapy, occupational therapy, speech, and vocational rehabilitation. Gouverneur is an academic affiliate of NYU School of Medicine and Rusk Institute of Rehabilitation Medicine.
Job SummaryUnder supervision, provides professional nursing care to patients/clients within an assigned unit. Diagnoses and treats responses to actual or potential health problems by means of nursing methods and techniques such as case-finding, health teaching, health counseling, and the provision of care supportive to and restorative of life and well-being.
Responsibilities- Plans, implements, and evaluates nursing care utilizing the nursing process.
- Formulates and implements comprehensive and nursing care plan. Sets priorities, delivers, and directs care to meet resident needs.
- Evaluates residents' responses and makes appropriate changes on the care plan, on an ongoing basis. Maintains accurate documentation as described in policies and procedures.
- Collaborates with other members of the interdisciplinary team in the care of residents, including the resident, family, and/or significant others. Discusses education and discharge planning issues as they arise.
- Transcribes physicians' orders and administers medication/treatment as defined in all policies.
- Assumes charge nurse responsibilities as assigned.
- Maintains professional competence through participation in continuing education and other appropriate learning experiences. Attends all mandatory classes and participates in the orientation/training staff.
- Demonstrates safety awareness and follows protocols for fire, disaster, infection control, and other safety practices.
- Participates in performance improvement activities and other committees as assigned.
- Performs other related duties.
- Supervises and directs Licensed Practical Nurse, Medical Surgical Technician, Certified Nurse's Aide, or other allied nursing personnel concerning the care of residents.
- A valid New York State license and current registration to practice as a Registered Professional Nurse in New York State.
- Valid and current certification in Basic Life Support (BLS) through the American Heart Association (AHA).
- A Bachelor's of Science degree in Nursing or higher (or other nursing degree, as identified by NYS Education Department including Master of Science in Nursing, Doctor of Nursing Practice, Doctor of Philosophy Degree in Nursing, or Doctor of Nursing Science) from an accredited college or university, registered with the NYSED.
