Key Responsibilities- Collaborate with clients and their families to develop and implement individualized treatment plans, focusing on vocational and educational objectives.
- Engage with employers and academic professionals to facilitate clients' access to employment and educational opportunities.
- Provide case management services, including operations support, and assist the therapy and nursing teams with case management tasks and unit operations tasks.
- Participate in weekly and other periodic supervision, NAVIGATE team meetings, and client treatment planning/review meetings.
- High School Diploma plus six (6) years of experience.
- Mental Health Paraprofessional certification within 90 days of hire.
- Bachelor's degree in education, English, Speech Communication, or related field plus two (2) years of behavioral health or other health care experience.
