Our team covers the Northern Nassau Area, providing skilled nursing care to patients in their homes. We offer a 5x 8 hour shift schedule, rotating weekends, and great healthcare benefits, including education reimbursement of $7,000 annually.

Job Details

As a Home Care RN with Catholic Health, you will:

  • Case manage, assess, plan, develop, and implement comprehensive home health care plans for patients.
  • Measure vital signs, administer IVs, change dressings, and clean wounds as needed.
  • Provide skilled nursing care, preventative rehabilitative procedures, and prescribed treatments for various patient populations in complex home situations.
  • Work closely with a supportive patient care team, including physicians, therapy, and social services.
  • Provide patient and family education for disease management and health promotion as directed by physicians' plans of treatment.
Requirements

To be successful in this role, you will need:

  • New York State RN license.
  • Minimum of one year of recent hands-on experience as a Registered Nurse.
  • Basic Life Support Certification.
  • Valid New York State Drivers License and reliable transportation.
Compensation

The salary range for this position is $95,000 to $125,000 per year, depending on qualifications, skills, and experience. In addition to the estimated base pay, Catholic Health offers generous benefits packages, tuition assistance, a defined benefit pension plan, and a culture that supports professional and educational growth.
