Join Catholic Health Home Care as a Registered Nurse and be part of a team that provides exceptional patient care. As a skilled nursing professional, you will work closely with a supportive patient care team, including physicians, therapy, and social services, to deliver high-quality care to patients in their homes.

**Responsibilities:**

* Assess, plan, develop, and implement comprehensive home health care plans for patients
* Measure vital signs, administer IVs, change dressings, and clean wounds as needed
* Provide skilled nursing care, preventative rehabilitative procedures, and prescribed treatments for a variety of patient populations
* Work closely with the patient care team to ensure seamless care delivery
* Provide patient and family education for disease management and health promotion

**Requirements:**

* New York State RN license
* Minimum of one year of recent hands-on experience as a Registered Nurse
* Basic Life Support Certification
* Valid New York State Driver's License and reliable transportation
