We are seeking a skilled Registered Nurse to join our team as a Home Health Nurse Specialist. This role will involve administering skilled care visits to clients requiring intermittent professional services, teaching the client, family, and other members of the health care team, and ensuring that all care is delivered in accordance with the physician's orders and the established plan of care.

Job Overview

As a Home Health Nurse Specialist, you will work closely with our interdisciplinary team to provide high-quality patient care in the comfort of their own homes. Responsibilities

The successful candidate will be responsible for:

  • Administering skilled care visits to clients, including medication management, wound care, and education on disease management;
  • Teaching clients and their families how to manage their conditions, use medical equipment, and perform daily living activities safely and effectively;
  • Collaborating with the interdisciplinary team to develop and implement comprehensive care plans tailored to each client's needs;
  • Maintaining accurate records and documentation of client care and progress;
  • Providing emotional support and guidance to clients and their families during times of need;
  • Participating in ongoing education and training to stay up-to-date on best practices and industry developments.
Qualifications

To be considered for this role, you must have:

  • A current and unencumbered Registered Nursing License in the state where you intend to work;
  • At least 1 year of experience in home health care or a related field;
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations;
  • Strong problem-solving and critical thinking skills, with the ability to prioritize tasks and manage multiple responsibilities;
  • A valid driver's license and reliable transportation;
  • Basic life support certification (BLS).
