Physical Therapist for Skilled Nursing Facility

We are seeking a highly skilled Physical Therapist to join our team at Carnegie Post Acute Skilled Nursing at Princeton. As a Physical Therapist, you will play a vital role in helping our patients achieve their rehabilitation goals and regain their independence.
Responsibilities:- Conduct thorough evaluations and assessments of patients to identify their physical therapy needs.
- Develop and implement individualized treatment plans to address patients' physical therapy goals.
- Provide hands-on therapy sessions to patients, including exercises, stretches, and other interventions to improve their mobility and function.
- Collaborate with other members of the healthcare team to ensure comprehensive care and coordination of services.
- Document patient progress and outcomes in a timely and accurate manner.
- Graduate of an accredited Physical Therapy program.
- Current licensure as a Physical Therapist in the state of New Jersey.
- Experience working in a long-term care setting, preferably in a skilled nursing facility.
- Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
