Physical Therapist for Skilled Nursing Facility

Carnegie Post Acute Skilled Nursing at Princeton is seeking a skilled Physical Therapist to join our team. As a Physical Therapist, you will work collaboratively with our therapy teams and Rehab Department to provide high-quality care to our patients.
Responsibilities:- Evaluate patients and develop treatment plans to improve their functional abilities.
- Provide physical therapy treatments, including exercises, stretches, and other interventions, to help patients recover from injuries or illnesses.
- Document patient progress and communicate with other healthcare professionals to ensure seamless care.
- Work independently to manage workflow, prioritize tasks, and meet deadlines.
- Familiarize yourself with PDPM and align therapy treatments and plans with understanding of Medicaid and Medicare billing.
- New Jersey License as a Physical Therapist.
- Experience in long-term care settings, preferably in a Skilled Nursing Facility.
- Strong organizational, interpersonal, and communication skills.
- Ability to manage patients with different personalities and ability levels.
