JOB OVERVIEW:
Under the general supervision of the Clinical Administrator, the licensed nursing professional is responsible for delivering health assessments and support services to participants in the program. This role involves monitoring participants' health conditions, administering prescribed medications, and assisting with daily living activities such as bathing, toileting, and eating. Additionally, the nurse provides health education and counseling to participants and their families, serves as a liaison with the participants' healthcare providers, and assists in accessing medical services from external providers. Home visits are an essential component of the ongoing projects.

KEY RESPONSIBILITIES:

  • Conduct initial nursing assessments and ongoing evaluations of participants; monitor health issues and develop nursing care plans.
  • Regularly monitor vital signs for each participant, including blood pressure.
  • Assist participants in achieving optimal health by coordinating health screening programs, administering prescribed medications, and providing first aid as necessary.
  • Offer health counseling to participants and their families, both individually and in groups.
  • Design and deliver health-related training for staff, volunteers, and families.
  • Refer participants and their families to suitable community health agencies or facilities; act as a liaison with these organizations.
  • Oversee personal care activities for all participants and supervise health care aides in their duties.
  • Act as a nursing consultant for staff and primary liaison with participants' healthcare providers as appropriate.
  • Maintain accurate and current written case management records; prepare various reports and correspondence.
  • Participate as a member of the Multidisciplinary Team in initial assessments, care planning, and ongoing evaluations of participants' care.
  • Assist the Nursing Supervisor in implementing Infection Control policies and procedures within the organization.
  • Collaborate with the Consultant Pharmacist for quarterly visits and follow up on recommendations.
  • Engage in daily program activities such as conducting classes, assisting with meals, and cleanup; may transport participants as needed.
  • Perform other duties as assigned by the supervisor.

QUALIFICATIONS:

  • Completion of an accredited nursing program with a current license to practice as a Registered Nurse in California.
  • Experience working with elderly, frail, or disabled adults.
  • Demonstrated flexibility, initiative, creativity, and organizational skills.
  • Bilingual in Chinese/English or Vietnamese/English is strongly preferred.

SALARY RANGE: The target hourly rate for this position is $49.00, with benefits. Compensation will be determined based on the candidate's education, experience, and skill set.
