WORK HOURS: Full-time (40 hours/week) or Part-time

STATUS: Non-Exempt (3 months orientation period)

JOB SUMMARY:
Under the general supervision of the Clinical Administrator, the licensed nursing professional is expected to deliver health assessments and support services for participants engaged in the program. This role involves monitoring participants' health conditions, administering prescribed medications, and assisting with daily living activities such as bathing, toileting, and eating. The nurse also provides health education and counseling to participants and their families, serves as a liaison with participants' physicians, and aids in securing medical services from other providers. Home visits are a necessary component of the ongoing projects.

KEY RESPONSIBILITIES:

  • Conducts initial nursing assessments and reassessments of participants; monitors health issues and formulates and implements nursing care plans.
  • Regularly monitors vital signs for each participant, including blood pressure.
  • Assists participants in achieving optimal health, coordinating and overseeing health screening initiatives, aiding with prescribed medications, and providing first aid as required.
  • Offers health counseling to participants and their families on both individual and group levels.
  • Develops and delivers health-related training for staff, volunteers, and families.
  • Refers participants and their families to suitable community health care agencies or facilities; acts as a liaison with these organizations.
  • Responsible for coordinating personal care activities for all participants and supervising health care aides in personal care tasks.
  • Acts as a nursing consultant for staff and as the primary liaison with participants' physicians as necessary.
  • Maintains current and accurate written case management records; prepares various written reports and correspondence.
  • Participates as a member of the Multidisciplinary Team in initial assessments, care planning, and ongoing evaluations of participants' care.
  • Assists the Nursing Supervisor in implementing Infection Control policies and procedures in the Center.
  • Liaises with the Consultant Pharmacist for quarterly visits and takes follow-up actions based on the consultant's recommendations.
  • Engages in daily program activities such as conducting classes, assisting with meals and clean-up, etc.; may transport participants to and from the center or to medical appointments on a relief or emergency basis.
  • Other duties as assigned by the supervisor.

QUALIFICATIONS:

  • Successful completion of an accredited nursing program with a current license to practice in California as a Registered Nurse.
  • Experience working with frail seniors or disabled adults.
  • Possesses flexibility, initiative, creativity, and organizational skills.
  • Bilingual in Chinese/English or Vietnamese/English is strongly preferred.

SALARY RANGE: The target hourly rate for this position is $49.00 with benefits. The specific compensation will be determined based on the education, experience, and skill set of the individual selected for this role.
