Job Title: Field HSS Coordinator

Key Responsibilities:
  • Provide evidence-based education to promote healthy pregnancy, delivery, and postpartum outcomes for mothers and babies.
  • Create a positive experience and relationship with members and your team.
  • Proactively engage members to manage their own health and healthcare.
  • Support social determinants of health needs.
  • Perform outreach to providers, hospitals, members, and clinics.
  • Attend community events.
  • Able and willing to travel within the assigned region per business needs.

Requirements:
  • Bachelor's Degree in health, human, social work, or education services with 1+ years of qualifying experience in Care Coordination of individuals with complex health conditions, including Care Coordination of Behavioral Health conditions or maternal and child health care experience.
  • Licensed practical nurse (LPN) in Kansas with 2 years of qualifying experience working within the maternal and child health population.
  • 1+ years of experience with MS Office, including Word, Excel, and Outlook.
  • Located in the state of Kansas.
  • Ability to travel up to 50% of time in assigned region to visit Medicaid members in their homes and/or other settings.
Preferred Qualifications:
  • Knowledge of the culture and values of community.
  • Case Management experience.
  • Background in Managed Care.
