Hospice RN Job Summary:

We are seeking a skilled RN Case Manager to join our Hospice team in Southaven, MS. This part-time position will work Monday, Wednesday, and Friday from 8:00am to 4:30pm. As a Hospice RN, you will provide end-of-life care to patients and their families, developing trusting relationships and delivering high-quality care. You will be responsible for evaluating patient needs, creating and implementing care plans, and collaborating with the interdisciplinary team. This is a rewarding opportunity to make a meaningful difference in the lives of others.

Responsibilities:

* Conduct initial nursing evaluations to determine patient eligibility for hospice services
* Identify patient and family needs, and re-assess as needed
* Develop and implement individualized care plans
* Collaborate with the interdisciplinary team to provide comprehensive care
* Maintain accurate and relevant clinical notes

Requirements:

* One year of clinical experience as an RN
* Current RN licensure in the state of practice
* Current CPR certification
* Reliable transportation and valid driver's license
