Position Summary: The Patient Care Assistant plays a vital role in supporting basic patient care activities.

Key Responsibilities:

  • Assist with care plans and procedures as directed by a Registered Nurse (RN).
  • Support patients with personal care, daily living activities, and safe transfers.
  • Monitor and report any changes in patients' physical, mental, or emotional conditions.
  • Perform light housekeeping tasks to maintain a clean environment.
  • Address patients' psychosocial and spiritual needs, ensuring comfort and dignity.
  • Foster and maintain positive relationships with patients, families, and team members.
  • Respond promptly to patient needs and safety concerns.

Qualifications:

  • Must hold a valid credential as a Companion, Home Health Aide, Nurse Aide, or Certified Nurse Aide.

Education & Experience:

  • High School Diploma or equivalent.
  • Prior experience as a Certified Nursing Assistant (CNA) is preferred.
