Job Summary

As a Social Services Case Worker at Orlando Senior Health Network, you will serve as a vital link between our residents and other departments, assisting them in adjusting to their new environment and lifestyle. Your role will involve coordinating family meetings, utilizing community services and resources, and advocating for residents and their families.

Key Responsibilities
  • Develop one-to-one relationships with residents and families to provide counseling and support.
  • Counsel residents and families on dealing with feelings about death or dying, emotional, mental, environmental, or physical limitations.
  • Advocate for residents and their families in expressing, defining, and resolving grievances.
  • Maintain documentation of all grievances for residents on 3rd and 4th floor.
  • Document all interactions with residents and/or family in the assessment and care plan as required by state standards.
  • Complete the social service portion of the MDS and RAPS per Medicare state guidelines.
  • Document the social care component of the Comprehensive Care Plan for each resident as identified by the interdisciplinary team per Medicare state guidelines.
  • Complete and review SNF Determination on Continued Stay with resident and/or responsible party when Medicare coverage ends.
  • Participate in discharge planning process and generate written discharge summaries to party responsible for patients health care at discharge.
  • Document the social and emotional needs of residents.
  • Work with Billing Coordinator and follow up with resident and family or responsible party on pending Medicaid applications.
  • Act as a liaison with social, health and community agencies.
  • Responsible for referral of residents to social, health and community agencies and ensure documentation in the resident's record concerning the results of such referrals.
  • Communicate available community resources to residents and their families.
  • Participate in inservice training programs with family support group.
  • Provide new hire orientation and ongoing in-services on resident rights, dignity issues, abuse, neglect, exploitation, resident grievance policy, advance directives, and behavioral issues.
  • Explain resident's rights to residents and their families.
  • Organize and participate in the theft/loss prevention program.
  • Coordinate with Activity Department to meet socialization needs of the resident.
  • Serve as abuse/neglect investigative officer with Risk Management.
  • Coordinate psychological services and maintains psychotropic log.
  • Perform other duties as assigned.
