AdventHealth Medical Group is seeking a skilled Clinical Social Worker to join our team in an outpatient setting. As a Clinical Social Worker, you will be responsible for assessing patients' physical, psycho-social, spiritual, and financial factors to identify barriers to goal attainment.
Key Responsibilities:
- Utilize expertise in patient assessment to identify barriers to goal attainment
- Order and interpret diagnostic tests, such as labs and imaging
- Monitor patients' course of treatment and make referrals to other specialists as needed
- Collaborate with physician supervisor to provide consistent care to patients and support to staff
- Document patient information in a timely and thorough manner
Requirements:
- Master's degree in Social Work or related field
- Geriatric psychiatry experience preferred
- Current, valid Florida LCSW or LMHC license
- BLS & ACLS certification
