Social Worker

We are seeking a highly skilled and compassionate Social Worker to join our team at Nemours Children's Health. As a key member of our healthcare team, you will play a vital role in providing comprehensive psychosocial services to patients and their families.
Key Responsibilities- Assessment and Intervention: Conduct thorough assessments of patients and their families' biopsychosocial situations, establish clear goals, and provide evidence-based interventions to address their needs.
- Crisis Intervention and Support: Provide timely crisis intervention services, including cases involving domestic violence, psychiatric emergencies, child abuse, and family disputes.
- Referral and Triage: Assess patient mental health needs and refer for counseling and psychiatry as needed, both internally and to external community providers.
- Comprehensive Care Management: Perform admission screenings, offer case management, medical crisis counseling, patient/family education, advocacy, residential placement, community referral facilitation, crisis intervention, and mental health evaluation.
- Child Abuse Investigations and Domestic Violence Management: Assist with the assessment and coordination of child abuse investigations within the hospital and manage domestic violence situations in collaboration with the medical team.
- Professional Development: Continuously update your education and skill level in the field, attend in-service presentations, and participate in continuing education programs to maintain and enhance your knowledge base.
- Hospital Discharge Planning: Assess social barriers to discharge, work collaboratively with case management, participate in rounds, and escalate complex cases to avoid discharge delays.
- On-Call Responsiveness: Promptly respond to events and service requests within 5 minutes of receiving a phone call and remain within a 30-minute drive of the hospital.
A Master's degree in Social Work from a program accredited by the Council on Social Work Education (CSWE) is required.
