Key Responsibilities- Build strong relationships with patients and their caregivers, providing personalized support and guidance throughout their treatment journey.
- Participate in patient evaluation and chart review, ensuring accurate and effective treatment plans.
- Collect, interpret, and evaluate physician medication orders, ensuring therapeutic appropriateness, cost-effectiveness, accuracy, safety, and completeness.
- Monitor patient progress and assessment results, communicating clinical and technical information with physicians and other healthcare team members.
- Directly supervise pharmacy technicians and support personnel, ensuring the proper compounding, packaging, and labeling of compounded sterile products.
- Assist in training and orientation of new employees, performance improvement activities, and educational programs.
- Bachelor's or Doctor of Pharmacy degree in Pharmacy from an accredited college or university.
- Current license to practice pharmacy in South Carolina.
- One year of related experience in an IV alternate site environment or equivalent preferred.
- Home infusion therapy or home infusion rotation experience preferred.
- Demonstrated experience in research or other areas of pharmacy.
- Current CPR certification if in direct contact with patients.
- Willingness to learn the clinical and business needs of Coram.
