About the Role

Nevada Health Centers, Inc. is seeking a highly motivated and compassionate Social Services Coordinator to join our team. As a key member of our healthcare team, you will play a vital role in ensuring our patients receive the best possible care.

Key Responsibilities
  • Assess patient needs and develop personalized care plans to address medical, psychosocial, and other social needs.
  • Collaborate with healthcare providers and other members of the patient's care team to evaluate progress and make revisions to care plans as needed.
  • Provide education and support to patients and their families to promote healthy behaviors and improve health outcomes.
  • Develop and maintain relationships with community resources and organizations to ensure patients have access to necessary services and support.
Requirements
  • Bachelor's degree in Health and Human Services-related field or equivalent experience.
  • Current Nevada State Social Work Licensure (LSW) preferred.
  • Minimum one year of experience in an outpatient or inpatient behavioral health setting preferred.
  • Bilingual Spanish and English preferred.
