As a vital member of the healthcare team, the individual will work diligently to ensure safety, adherence to best practices, and the maintenance of high-quality care standards throughout the continuum. Responsibilities include orchestrating a variety of self-management support and disease registry initiatives for the identified population, utilizing data to manage health outcomes effectively.
Close collaboration with physicians, administration, and staff across all facilities is required to review medical charts, provide insights on quality incidents, and present data that supports the overall quality of care. A key characteristic for success in this role is exceptional interpersonal customer service, characterized by a positive attitude, a warm and friendly demeanor, and a deep respect for patients, families, colleagues, and all stakeholders.
EDUCATIONAL REQUIREMENTS- Education: Registered Nurse credential required; Bachelor of Science in Nursing preferred.
- Experience: A minimum of three (3) years of experience in a hospital setting.
- Licensure: Current Washington State RN license is mandatory.
- Additional Skills: Bilingual abilities, particularly in English and Spanish, are preferred. Experience working with diverse populations is advantageous.

Compensation Range: $54.03 per hour
