Job Title: Patient Care Coordinator

Oncology Specialists of Charlotte is a leading medical oncology practice dedicated to providing high-quality, compassionate care to patients with cancer and blood disorders.

We are seeking a highly skilled Patient Care Coordinator to join our team. As a Patient Care Coordinator, you will play a vital role in ensuring the smooth operation of our practice, providing exceptional patient care, and supporting our healthcare team.

Key Responsibilities:
  • Answer incoming calls in a prompt, courteous, and professional manner
  • Route calls and messages appropriately, following standard operating procedures (SOPs)
  • Present a personable and resourceful approach to problem-solving, providing support and help to all callers
  • Greets patients/visitors and provides assistance as needed
  • Obtains pre-patient registration, demographic, and insurance information, and enters it into our Patient Management System (PM) or Electronic Health Record (EHR)
  • Requests, collects, documents, and assesses all necessary records to ensure patient scheduling and continuity of care
  • Communicates with outside entities to obtain patient medical history records when necessary
  • Ensures all necessary medical information is obtained and entered into patients' electronic medical records ahead of appointments
  • Runs and/or works daily reports to identify patients with unscheduled orders
Quality Initiative Programs:
  • Ensures communication is distributed to patients regarding program details
  • Generates and distributes Clinical Summaries from Electronic Health Records (EHR) to external providers
  • Communicates information regarding Patient Engagement Portals and assists with registration processes
