Key Responsibilities

The Palliative Care Nurse Practitioner/APRN will deliver direct care to patients and their families through specialized palliative care consultations, ensuring that patients with serious illnesses and limited prognoses receive appropriate services.

  • Exhibit ethical and effective application of evidence-based palliative medicine practices in all aspects of patient care, considering physical, emotional, spiritual, psychosocial, ethical, educational, and safety factors relevant to the patient demographics.
  • Utilize clinical expertise to integrate palliative care knowledge in developing effective care plans.
  • Systematically collect relevant information from diverse sources, including medical records and family histories, while conducting thorough assessments of physical, emotional, psychosocial, and spiritual health.
  • Perform comprehensive evaluations using established palliative care assessment domains, interpret findings, conduct medication reconciliations, develop problem-oriented diagnoses, initiate care plans with therapeutic interventions, and assess the outcomes of these interventions.

Qualifications

  • Current RN/APRN licensure in the state of practice
  • Nurse Practitioner/CNS with prescriptive authority, maintaining all licensure requirements
  • A minimum of 5 years of clinical nursing experience, including at least 1 year as an APRN in Hospice, Palliative Care, or Oncology settings preferred
  • Advanced practice palliative care certification (ACHPN) is preferred; certification in a relevant advanced practice area will be considered. BCLS certification is required, and ACHPN certification is encouraged within two years of employment or sooner if experience requirements are met.
