Key Responsibilities:
The primary role involves the effective management and transition of participants in our care programs, ensuring seamless communication and advocacy for optimal healthcare delivery. The Nurse Manager will play a crucial role in:

  • Coordinating care for participants admitted to various healthcare settings.
  • Facilitating communication among multidisciplinary teams to ensure comprehensive care.
  • Acting as an advocate for participants, ensuring their needs are met throughout their healthcare journey.

Qualifications:
To be successful in this role, candidates should possess:

  • A Bachelor's degree in Nursing is preferred; a graduate from an accredited nursing program is required.
  • A valid Registered Nurse (RN) license in the state of employment.
  • A minimum of two years of nursing experience in acute care, nursing home, or community-based settings, with a preference for experience in geriatric care.
  • Experience in case management and working collaboratively with healthcare teams.
  • Experience working with elderly populations or completion of relevant training within the first six months of employment.
  • A valid driver's license and reliable transportation.
