About the Role

The Laurels of Kettering is seeking a skilled and experienced Clinical Nurse Specialist to join our team as an MDS Nurse. In this role, you will be responsible for conducting assessments and care plan coordination for residents assigned to you.

Key Responsibilities
  • Assessment and Care Planning: Complete the Minimum Data Set (MDS), Care Area Assessments (CAAs), and care plans within regulated time frames.
  • Resident Assessment: Assess residents through physical assessment, interview, and chart review.
  • Collaboration and Communication: Discuss resident care needs with caregivers, including physicians, nurses, social services, therapy, dietary, and activity staff.
  • Information Gathering and Review: Review information from hospitals, consults, and outside agencies and use such information in the completion of the assessment and care planning.
    • Significant Change MDS: Coordinate, identify, and/or initiate significant change MDS.
    • PPS Meetings and Monitoring: Conduct PPS meetings and maintain MDS assessments per Medicare schedule, and monitor Medicare days and RUGs utilization in the absence of the Care Management Coordinator.
  • Professional Development: Remain current with the American Association of Nursing Assessment Coordinators (AANAC) requirements.
Requirements
  • Education: Registered Nurse (RN) degree.
  • Certification: AANC certification (RAC-CT) is a plus.
  • Experience: Experience as an MDS Nurse.
  • Knowledge: Knowledge of the Resident Assessment Instrument (RAI) process, including the principles of the Prospective Payment Process (PPS), is strongly preferred.
