Key Responsibilities

  • Conduct comprehensive assessments of patients to identify their unique needs and develop personalized care plans.
  • Collaborate with our interdisciplinary team, including physical therapists, occupational therapists, and social workers, to ensure seamless care coordination.
  • Implement and drive nursing clinical programs and initiatives to achieve desired outcomes, leveraging technology and telehealth to advance patient goals and outcomes.
  • Supervise a caseload of licensed practical nurses and home health aides, ensuring high-quality care and facilitating care coordination with PRN staff as needed.
  • Document patient information accurately and timely using a tablet, including OASIS assessments, SOC admissions, recertification, and physician orders.
  • Facilitate care collaboration and coordination with primary and specialty healthcare providers.
  • Participate in team and case conferences, leading discussions on multi-disciplinary cases as needed.

Requirements

  • Active RN license
  • One year of clinical experience as a Registered Nurse (RN) required
  • Valid driver's license
  • Intuitive knowledge of technology and comfort with smartphones, tablets, virtual reality, interactive software, and artificial intelligence
  • Open and adaptive to change
  • Strong communication skills
