Responsibilities
  • No on-call requirement and flexible schedules to ensure work-life balance.
  • Conduct comprehensive patient assessments and collaborate with interdisciplinary teams to achieve patient goals and outcomes.
  • Implement and drive nursing clinical programs and initiatives to improve patient outcomes, leveraging technology and telehealth to advance patient care.
  • Supervise a caseload of LPNs and Home Health Aides, facilitating care coordination with PRN staff as needed.
  • Document patient information accurately and timely using a tablet, including OASIS assessments, SOC/Admissions, recertification, and physician orders.
  • Care Collaboration/Coordination with primary and specialty healthcare providers.
  • Participate in collaborative Team/Case Conferences, leading discussions on multi-disciplinary cases as appropriate.
Qualifications
  • Active RN License.
  • One year of clinical experience as a Registered Nurse (RN) required.
  • Valid Drivers License.
  • Intuitive knowledge of technology and comfort with smartphones, tablets, virtual reality, interactive software, and artificial intelligence.
  • Open and adaptive to change.
  • Strong Communication Skills.
  • Passion for learning.
  • In Washington State, all providers must meet NPI requirement and furnish proof prior to onboarding.
