Job Title: Travel Nurse Provider

We are seeking a highly skilled Travel Nurse Provider to join our team at WelbeHealth. As a Travel Nurse Provider, you will play a critical role in delivering high-quality care to our seniors in their homes and communities.

Job Summary:

The Travel Nurse Provider will work collaboratively with our interdisciplinary team (IDT) to provide comprehensive assessments, develop care plans, and coordinate care for our participants. This is a unique opportunity to apply your clinical expertise in a new and innovative way, while making a meaningful difference in the lives of our seniors.

Responsibilities:
  • Perform in-person comprehensive assessments, including initial, semi-annual, and unscheduled assessments, to ensure the health and well-being of our participants.
  • Collaborate with physicians and the IDT to develop and implement care plans that meet the unique needs of each participant.
  • Work closely with the IDT to coordinate care and ensure seamless transitions between settings.
  • Provide primary care and continuing medical education to our participants, as needed.
  • Travel to our various centers to provide care and support to our participants.
  • Review outcomes and utilization data to identify opportunities for improvement and develop best practices.
  • Participate in rotating night and weekend call to ensure 24/7 coverage.
Requirements:
  • Graduation from an accredited school of nursing.
  • Certificate as a Nurse Practitioner or Physician Assistant with prescriptive authority.
  • RN state license.
  • Minimum of two years of experience working with the senior population.
  • Current knowledge of chronic care and geriatric issues, as well as best practices.
  • Reliable means of transportation.
